WAIVER AND RELEASE

I HEREBY DECLARE that I am the legal owner of my dog and that my dog has not been exposed to distemper, rabies, parvovirus or any other contagious disease within the past thirty (30) days and that my dog has been vaccinated by records presented.

My dog’s boarding is not without risk. I authorize Paradise Kennels Ltd. to do whatever they deem necessary for the safety, health and well-being of my dog while under the care of Paradise Kennels Ltd. including seeking professional treatment for my dog. Every effort will be made to provide a safe environment for your dog. If your dog requires medical attention, we will contact you immediately and make arrangements with you. If we cannot reach you, we will transport your dog to the Emergency Animal Clinic if we deem any injury is life threatening. I certify that all known medical and behavioral conditions are listed and I will not hold the kennel liable for these conditions or for injuries due to stress related behaviors (ie. scratching, chewing, pacing ect…) . If my pet is injured or becomes ill while boarded, I authorize the kennel to provide first aid or seek veterinary attention and understand that the cost for these services and any medication will be added to my invoice. I permit Paradise Kennels Ltd. staff to administer pet first aid if any injury is deemed none life threatening.

I am solely responsible for any damage or injury incurred while my dog is boarded, to include payment of costs of injury to staff, other clients and other animals or damage to facilities caused by my dog. I waive any and all claims, actions or demands of any nature, foreseen or unforeseen, that I may have against Paradise Kennels Ltd. relating to the care, control, health and/or safety of my dog arising during pick-up, transportation, drop-off and stay at the facilities. Paradise Kennels staff have the right to choose whether it is safe for your dog to partake in the various activities including but not limited to: playing with other dogs, going on walks on or off leash, swimming etc… I understand my dog’s acceptance into boarding is discretionary and that Paradise Kennels Ltd. has the right to refuse any service to me and/or my dog at any time for any reason.

I understand that if my dog has a history of or repeatedly demonstrates aggression or biting humans or animals, Paradise Kennels Ltd reserves the right to refuse service. I, the undersigned, agree to the above, accept full responsibility for the acts of my dog while being boarded and release Paradise Kennels Ltd., and its owners and staff of all claims of liability.

I acknowledge that I have carefully read all parts of the Agreement, understand its contents, and understand that this agreement includes an assumption of the risk of Paradise Kennels Ltd. negligence and liability.

I acknowledge that Paradise Kennels Ltd. is materially relying on this waiver and is allowing my dog(s) to engage in play with other dogs, go on off leash walks, play in play areas and the facility during their stay in the Paradise Kennels Ltd boarding facility. If the dog becomes ill or if the state of the dog’s health otherwise requires professional attention, the Kennel, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the dog, and the expenses thereof shall be paid by Owner. This contract contains the entire agreement between the parties.

All terms and conditions of this Contract shall be binding on the heirs, administrator, personal representatives and assigns of Owner and Kennel. Any controversy or claim arising out of this Contract, or the breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this Contract, shall be settled by arbitration.  I have read all of the above and agree to comply.
